PLEASE BE PREPARED TO SUBMIT REQUIRED DOCUMENTATION AND READ ALL OF THE BELOW INFORMATION PRIOR TO PROCEEDING WITH THE APPLICATION. NO REFUNDS WILL BE GIVEN FOR AN INCOMPLETE APPLICATION. A NONREFUNDABLE RENTAL PROCESSING FEE OF $35.00 PER APPLICANT MUST BE SUBMITTED WITH THIS APPLICATION. ALL OCCUPANTS 18 YEARS OF AGE AND OLDER MUST COMPLETE AN APPLICATION. APPLICATIONS WILL NOT BE FULLY REVIEWED WITHOUT A COMPLETE APPLICATION & FEE, INCLUDING ALL REQUIRED DOCUMENTS AS OUTLINED BELOW. APPLICATION FEE IS USED TO RESEARCH CREDIT SCORES, CRIMINAL BACKGROUND AND NATIONAL EVICTION RECORDS. ANY FALSE, MISLEADING OR FRAUDULANT INFORMATION IS GROUNDS FOR IMMEDIATE DENIAL.

A MINIMUM CREDIT SCORE OF 550 IS REQUIRED

AVAILABLE INCOME MUST BE TWICE THE RENT AMOUNT (take home income LESS monthly reoccurring debt/bills = AVAILABLE INCOME)

Any active summary possession or debt collection relating to prior or current rental units is grounds for immediate denial. Any unpaid judgement relating to another rental unit within the last 10 years is grounds for immediate denial. Any summary possession or eviction of record occurring within the last 5 years may be grounds for immediate denial. Applicants are encouraged to fully disclose prior evictions and provide additional explanation for the cause of such eviction.

REQUIRED DOCUMENTATION includes but may not be limited to:

• Copy of Federal or State ID

• Copy of Income Verification Documents such as paystubs, tax returns for self-employed, letter/statement(s) from Federal or State Subsidy or Benefit programs, retirement statements or the like. Please provide written documentation for ALL sources of income/benefits. Verbal Verification is not acceptable.

• Bank statements or other financial institution statements (i.e. financial “apps” such as Cash App, Venmo or payroll debit cards…) showing income deposits. All statements, screen shots, transaction history statements must show the name on the account and the date of the statement.

• Written prior Landlord Reference for the most recent property rented by applicant. Providing accurate contact information for the prior landlord is satisfactory and we will contact them to complete a questionnaire. If no prior Landlord Reference is available due lack of rental history, application must include personal references and contact information.

5 Star Property Management utilizes the recommendation provided by Propertyware Screening along with information provided by required documentation to determine the sustainability of applicant's ability to afford rent, utilities and applicant's other financial obligations as disclosed on the PW Screening Report and income statements.

As part of our application process, we require ALL applicants to complete a PetScreening profile, regardless of whether or not you have a pet, a support animal, an assistance animal or no animals at all. PetScreening.com is an independent 3rd party service where applicants will answer questions relating to their animals and submit any applicable documentation that may be required. There is no charge for profiles for “No Pet or Animal” or for “Assistance Animals”. There is a fee of $20 (ACH) or $25 (all other forms of payment) per pet for a “Household Pet” profile. 5 Star Property Management will receive your profile summary upon completion of your profile. Please note there are a few breed restrictions for PETS. Breed restrictions include: Doberman Pinschers, Pit Bulls (Pit Bulls as defined by insurance companies generally encompass: Bull Terriers, Staffordshire Bull Terriers, American Bull Terriers, and American Staffordshire Terriers or any combination of these breeds), Rottweilers, Chow Chows, Wolf dogs (and wolf hybrids), Presa Canarios (Canary Dog), and Akitas.
